Morton Freshman Center Incoming Freshman Enrollment Information

Families that have just moved into Cicero and/or prospective students for the 19-20 school year that are not graduating from Unity Jr. High must call our Operations Department and make an enrollment appointment at 708-780-4000 ext. 2327 or ext. 2329. Familias que se han mudado al area de Cicero o futuros estudiantes para el año escolar 17-18 que no se van a graudar de Unity Jr. High deben llamar y hacer una cita para inscribir a su hijo/a en el Distrito 201 al 708-780-4000 ext. 2327 o ext. 2329.

Health Services Office

All students must have a completed physical, health history update, and authorization for exchange of confidential information form approved by the Freshman Center Health Office.  A school physical form, health history update form and authorization for exchange of confidential information information form can be found on the right side under links. All completed forms can be dropped off during registration or in the Health Office starting on August 14th. Todos los estudiantes necesitan su actualizacion fisica, historia de salud y formulario de liberacion de informacion aprobado por la Oficina de Salud. Una copia del examen fisico, forma del historial medico y el formulario de liberacion de informacion se pueden encontrar en la lista de link al lado derecho de esta pagina. Todos los formularios completados se pueden entregar durante registracion o en la oficina de salud empezando el 14 de agosto.
